No, it is not incest. They’re just cousins. But, according to some system within the monarchy, people tend to marry the families they’re closely connected with and such all because they had to maintain their family status. It wasn’t exactly for love, but rather for political purposes.

Since I am pretty sure all kingdoms in the ancient period had inbreed marriages meaning you would marry someone who had close blood relationships to maintain your bloodline purity/status/political whatnot. There were even case of marrying siblings and daughters. Therefore Generally most nobles somehow related back to the king (tracing back to ancestry). I would write more but it would go on forever….
It doesnt seem like the late empress is related to the Fried extended family cuz it says that Dorothea was REMINDED of the Fried family while looking at her mom’s portrait, no one has said the late empress’s name so far.
AND the King wouldn’t NEED to get closer to the Fried family through marriage of his hated daughter with Theon, if the king himself was ALREADY married to a significant Fried family member.
So i just think she happens to resemble the Fried’s and that’s PART of why Dorothea likes Theon
